Freedom Pharmacy (HHCS Pharmacy, Inc. d/b/a) is dedicated to protecting its patients' privacy. Freedom Pharmacy is fully licensed and operating under State of Florida and federal laws. The records Freedom Pharmacy creates and maintains related to its patients and the dispensing of medications are medical records. Freedom Pharmacy maintains its patients' medical records in compliance with the many state and federal laws which protect the privacy of those records. Freedom Pharmacy cannot and will not give, sell, trade, rent, disclose, loan or make available any identifiable personal information about its patients to any unauthorized third party, without first receiving patient permission, except or unless the law requires us to do so. Consistent with privacy laws, personally identifiable information may be provided to patients' individual doctors or other health care providers, as well as to patients' insurance companies as a part of the billing process. This page will tell you what information is collected from you through the Freedom Pharmacy web site, how the information is used, with whom the information may be shared, the choices you have regarding the collection use and distribution of the information, and how you can correct inaccuracies in the information. HOW WE USE PATIENT INFORMATION

Your medical record, may be shared with your doctor to confirm your prescription, and certain information may be shared with your insurance company, if necessary, to process your claim(s). We cannot and will not share personally identifiable information with other third parties without your consent. The personally identifiable information provided by Freedom Pharmacy, or edited by you, will only be used in the processing of your order and in the update of your Personal Profile. As many other health care providers, pharmaceutical manufacturers, physicians, hospitals, and educational institutions) do, we may, from time to time, collect and share non-personal, aggregate statistical information regarding patient patterns or trends, sales, traffic patterns, and site usage with contract partners and sponsors. We are restricted by law from sharing this type of information in a manner that would enable a third party to determine individual patient identities. We are constantly monitoring the Freedom Pharmacy web site so that we can analyze our patients' needs and suggestions. We are able to track individual site users' progress through our site pages and do this to determine which pages or elements of our web site are effective or may not be meeting our patients' needs. This tracking function occurs when you enter our site by logging on to your Personal Profile page. At that point, a "cookie" is created and placed in your hard drive and the cookie tracks your use of our site. The cookie is able to transmit back to us your site use information. Once you close your web browser, the cookie simply terminates and is not permanently stored on either your computer or in our system. Despite this tracking ability, we do not create a record of site usage which is personal to any patient or which becomes a part of a patient's Personal Profile, medical record, or account information.

We provide links to other web sites that we believe can offer you useful information and services. These links function in much the same fashion as a referral to a useful magazine article or reference book. Just as we would not be responsible for the contents of another person's article, or actions of a third party author, we are not responsible for the privacy policies or information provided on linked sites. Further, we are not responsible for the actions of the persons who create and operate linked sites. You should refer to the privacy policies posted on the other sites, or contact the parties controlling the other sites for information about their information gathering processes and information use policies.

COMMUNICATIONS WITH PATIENTS

From time-to-time our site requests information from users via surveys . Participation in these surveys is completely voluntary and the user therefore has a choice whether or not to disclose this information. Information requested may include contact information (such as name and shipping address), and demographic information (such as zip code, age level and health information (such as allergies, reactions to medications) Survey information will be used for purposes of improving the use and satisfaction of this site and health of our patients. We may occasionally send individual patients site, medication and service announcement updates, by e-mail, but only if the patient agrees to receive them by opting in on the Personal Profile page. Patients may un-subscribe at any time from receiving medication and service announcements, which contain important information about the service and medications. We will communicate with patients via email or the telephone, at their instruction, to respond to questions regarding services, medications, or accounts.

NOTIFICATION OF CHANGES

In the event of changes to our privacy policy we will post those changes on our homepage. Patients who maintain a Personal Profile through our Web page will receive a notification via email if we propose to make any change in our use of personally identifiable records. Any such uses must comply with applicable laws and each affected patient will be given the option to accept or reject the proposed change in regard to the patient's own records. If a patient rejects such changes, use of the patient's records will be governed by the policy in effect when the patient first created his or her Personal Profile.
